Eden Adams Memorial Scholarship 
                    2022- 2023 Academic Year
  NC4K is pleased to offer the annual Eden Adams Memorial Scholarship. This scholarship is intended to benefit those
  affected by pediatric cancer by helping to fund their education, with a single or multiple recipients chosen each year
  based on applicants and our funding abilities. This scholarship is awarded  ONE TIME  for the 2022-2023 academic year
  in the amount of $1,500. Funds will be paid directly to the accredited institution; one half each semester. The NC4K
  Scholarship Review Committee has the sole discretion to make decisions regarding this scholarship, and all decisions
  are final.
  To be eligible for the Eden Adams Memorial Scholarship, one must:
  ● Have had a diagnosis of cancer before the age of 19.
  ● Live in Ohio or have sought treatment for cancer in Ohio.
  ● Have received support from NC4K or participated in NC4K events.
  ● Be a high school senior, high school graduate or current post-secondary undergraduate who pla ns  to enroll in
  full-time undergraduate study at an accredited two- or four-year college, university or vocational-technical school for
  the entire upcoming academic year  with a minimum grade point average of 2.5 on a 4.0 scale (or the equivalent).
  Required Documents:
  ● Completed application form.
  ● An official transcript of academic records from high school or college, whichever is applicable.
  ● An acceptance letter (on letterhead) from the academic institution you will be attending. If already enrolled,
  please provide a copy of your registration confirmation.
  ● An essay describing your life experiences, your future goals, and the reasons why you need and deserve this
  scholarship. This essay should be no more than 3 pages.
  ● A verification of diagnosis on letterhead from the applicant’s physician.
  ● Recommendation letter from an adult age 21 or older who is not a relative.
  The  Scholarship  Committee  will  evaluate  each  application  based  upon  the  essay,  academic  achievement,  and 
  involvement  in  the  community.  This  committee  will  convene  in  mid-June  2021  to  decide  the  recipient  for  the  2022-2023 
  academic  year.
  Applications must be submitted in a single packet of information online, via mail or by email. Please note: the official
  transcripts and recommendation letter may be submitted directly to NC4K using the information detailed below.

  The scholarship is intended to be offered on an annual basis and is  non-renewable , but NC4K reserves the right at any
  time for any reason to refrain from offering the scholarship in a particular year.
  For the 2022-2023 academic year, applications will be accepted until  July 16,2022 .
